Some stumpers I've had in the past: Erroneous gas gauge, '67 Galaxie: Bad instrument voltage regulator. Sticky throttle '72 Datsun 1600 pickup: Broken engine ground strap. The throttle cable was welding itself by carrying the current. '74 VW Dasher would get vapor lock after being driven at speed for some while: Clogged fuel filter. '84 Buick Century stalling: Fuel pump put out adequate pressure when cold (which was when two shops measured it), but would drop in 15 mins. (Computer kept indicating sticky EGR valve. $2K worth of repairs went into this before problem found.)

Why would the cam "gear" or "sprocket" go out on a 2005 Scion xB? It was replaced under warranty, but no one seems to have an answer for this. The dealer just looks at me like I don't have the proper security clearance to know this.

Did you change the oil regularly? The part may be plastic -- if you went drag racing or pulled a trailer in hot weather or other non-conventional use of the vehicle could do the trick.

Or the part was defective in the first place, which the dealer wont admit to, even if they knew it. Recalls are a factory MFGR problem, and this sprocket might be the subject of the next recall, but the dealer cant talk about it.
